Do not submit health information through the general contact form. The form is intended for general, nonclinical inquiries. Do not include symptoms, diagnoses, medications, treatment history, insurance information, laboratory results, images, or other health information. If you submit health information despite this instruction, we may use and protect it as appropriate under applicable law and Practice procedures.

2. Information Collected Automatically

The Site does not use advertising cookies or cookie-based analytics and does not knowingly track visitors across unrelated websites. The Site and its vendors may still process limited technical information necessary to deliver, secure, and maintain the Site, such as IP address, browser/device information, request logs, and security-event data.

3. How We Use Site Information

We may use Site information to respond to inquiries; manage event RSVPs; provide requested information; communicate with you where you have opted in or where otherwise permitted; operate, protect, troubleshoot, and improve the Site; comply with law; and establish, exercise, or defend legal claims.

We do not sell personal information. We may disclose Site information to vendors that provide services for the Site or our communications, including Lovable (website hosting), Google Workspace (email and contact-form delivery), iPlum (texting), and CharmHealth (scheduling and patient portal), but only as appropriate for their services and subject to applicable agreements and law. We may also disclose information when required by law or to protect rights, safety, security, or property.

5. Third-Party Platforms

If you select a third-party link or service—such as CharmHealth (scheduling and patient portal), maps, or social media—the third party's privacy practices govern the information you provide directly to it. Review those policies before submitting information.

6. SMS Privacy Statement

No mobile opt-in data or consent for text messaging will be shared with third parties or affiliates for their marketing or promotional purposes. We may use service providers to deliver messages on our behalf and to maintain consent and opt-out records.

Use of information collected: We may use the information we collect to perform the services requested including billing, customer service, appointment reminders and other administrative requests.

Sharing of information collected: Mobile information will not be shared with third parties/affiliates for marketing/promotional purposes. This includes text messaging opt-in data and consent.

As a current or prospective customer, you understand that you can text us STOP at any time to opt out of receiving SMS text messages from us. You can text us HELP at any time to receive help.

You understand that the messaging frequency may vary. Message and data rates may apply.

All policies are followed as per CTIA guidelines 5.2.1. At any time if you want your information to be removed, you can contact us via our email address or regular mail.

7. Your Choices

To request access to, correction of, or deletion of personal information submitted through the Site, contact patientsupport@nsgwellness.com or (757) 848-4777. We will evaluate requests consistent with applicable law, our legal obligations, and the nature of the information. To stop text messages, reply STOP or use another reasonable method to tell us you no longer wish to receive texts. For clinical records and protected health information, use the process described in our Notice of Privacy Practices.

8. Data Security and Retention

We use reasonable administrative, technical, and physical safeguards designed to protect Site information. No internet transmission or storage system is completely secure. We retain Site information only as long as reasonably necessary for the purposes described here, to meet legal obligations, resolve disputes, and enforce agreements.

9. Children

The Site is not directed to children under 13, and we do not knowingly collect personal information from children under 13 through the Site. If you believe a child has provided information through the Site, contact us at patientsupport@nsgwellness.com.

10. Cookies and Similar Technologies

The Site uses technical storage and processing needed to deliver, secure, and maintain the Site. The Site does not use advertising cookies, cookie-based analytics, or cross-site tracking.

When you choose to use a linked or embedded third-party service, such as CharmHealth (scheduling and patient portal), that provider may collect information or place technologies under its own privacy practices. The Practice does not control those third-party practices.
